Appendix 1 to the 2023 residence card – who does it apply to?

Appendix 1 to the residence card is an appendix that is attached to an application for a temporary residence permit in Poland where the purpose of the foreign national’s stay, who is submitting the application, is:

1. To carry out work;
2. To carry out work in a profession requiring high qualifications;
3. Carrying on a business;
4. Holding a position on the board of directors of a limited liability company or a public limited company which the foreign national has established or in which they have subscribed to or acquired shares;
5. Managing the affairs of a limited partnership or a limited joint-stock partnership as a general partner;
6. Acting as a proxy.

Appendix 1 – residence card – how to complete it?

Are you a foreign national applying for a temporary residence permit in Poland for the purpose of working, running a business or performing specific functions? Appendix 1 to the residence card is the document you need to complete. Below is a brief guide on how to do this.

In Appendix 1 to the application for a residence card, you must first provide the details of the entity entrusting you with work in Poland, including the legal basis for its operations (name and registration number in the relevant register).

Next, in the form, you must complete the section containing information about your employer.

You must also include detailed information in Appendix 1 to the residence card application regarding the work you will be carrying out, including in particular:

1. Job title / type of work;
2. Place of work;
3. Legal basis for the employment;
4. Working hours;
5. Gross salary (or, where applicable, gross hourly rate);
6. Scope of core duties;
7. Period of employment.

The next section of Annex 1 to the residence card consists of the relevant declarations regarding the absence of any final convictions for specific offences.

Finally, the date on which the annex to the residence card was completed must also be indicated, and the signature of the entity commissioning the foreign national to carry out the work must be obtained.
